**Please note that registration remains open. Registrations received after the 5/31 deadline will not include a personalized Camp tee shirt.

This registration session will allow you to complete contact information, sign up for participation, and submit payment.

The grade is based on your child's grade for the 2024-2025 School Year. Be sure to specify which school building you will be attending for the 2024-2025 School Year.

Please read through each page carefully to ensure that all information is provided accurately. 

To complete registration through our secure site, please have your Visa, MasterCard, Discover, or ACH Bank Account information available.

Please have the following information ready before filling out this registration:

1.  Do you already have a sportsengine.com account?  If your daughter played indoor field hockey for Pittsburgh Venom, then you have an account.  Please use it as it will expedite your registration process and auto fill in most of the required information.

If you do not have a sportsengine.com account, you will be required to set up an account with sportsengine before you begin summer camp registration.

2.  Player and Parent/Guardian Information Section:  Your player's 2023-2024 grade level, school name and a current USA Field Hockey Number. 

Please note - most of our families renew their USA Field Hockey membership each year just before camp.  Chances are that your membership is about to expire (or will during the upcoming season) and a new one is needed.  Families who have players participating in field hockey year-around (school and club) are most likely to have current memberships that do not expire during the upcoming season.

To check the status of your USA Field Hockey membership, find your membership number, and/or purchase a new membership, please visit  https://www.teamusa.org/usa-field-hockey/membership

 Membership is good for a year and covers your USA Field Hockey membership requirement when you play for your school and club teams and USA Field Hockey regional and national sanctioned events.

Please write down the number and have it ready for this registration.

3.  Emergency and Medical Contact Information Section:  Primary emergency contact information, Medical insurance company name, policy number and the name of the policy holder, Physician contact information, Hospital of choice and player medical history.

4.  Financial assistance is available for families who need it.  Please email us at mtlebanonfieldhockey@gmail.com to inquire about assistance.

Discount when signing up for both sessions. Each session is $135.  Both sessions for $225!

Session one:  $135
Mondays and Wednesdays  - June 10th, 12th, 17th, 19th, 24th & 26th

Session two: $135
Mondays and Wednesdays - July 22nd, 24th, 29th, 31st  August 5th & 7th

Times for both sessions:
Grades 3-8: 4:00-6:00
